The international Society of Advance Care Planning promotes the universal adoption of Advance Care Planning (ACP) and patient-centered care to enable the delivery of goal concordant care in situations the person cannot speak for themselves (including emergencies, ICU treatment, chronic incapacity of decision-making, and end of life care) taking into account each individual's physical, psycho-social and spiritual care needs.
